It can be especially difficult to utilize with a fidgety patient since any bodily movement can cause what looks like Denti Strength an "earth quake" in the mouth, when viewed through my loupes. This is probably the primary reason I haven't gone to a higher magnified pair of loupes or purchased an operatory microscope, since these earthquakes can be disconcerting and would certainly increase in severity with higher magnification.

Another drawback is they have a different depth of field than our regular vision. Taking instruments in and out of the mouth can present difficulties and a dentist using loupes usually looks through the non magnified part of his glasses when taking instruments in and out of the mouth.

At this point in my career I use these loupes for all procedures, and do not pick and choose which procedures require magnification. Aside from the fact that it can be annoying removing my glasses, all of the procedures I perform benefit from me using my magnifying loupes. Some dentists resist using loupes since they feel that they will become "dependent" on them and not be able to go back to how they used to perform procedures. This is probably true, but why would a dentist want to not use these valuable additions to our armamentarium!
